This Privacy Policy explains what information we collect when you visit our website or submit a form, how we use it, who we share it with, and how long we keep it. We aim for clarity over legalese.

Overview

Eminent Hospice Care Inc. operates this website to provide information about hospice services and to receive inquiries and patient referrals. We collect only the information you submit through our forms, and we use it only to respond to you or coordinate care. We do not sell your personal information, we do not run advertising trackers, and we do not use cookies on this site.

Information we collect

We collect personal information only when you submit it directly through one of our forms.

Contact form

When you submit our Contact form, we collect your name, email address, phone number (optional), the subject category you selected, and the message you wrote.

Referral form — callback request

When you request a callback through our Referral page (the default mode), we collect only the referrer's name, phone number, optional email, preferred callback time, and referrer type (physician, family member, case manager, or other). We do not collect any patient health information through this form. Patient information is taken by phone.

Referral form — full intake (when enabled)

When the full Referral intake is enabled, we collect patient identifiers (legal name, date of birth, primary diagnosis, current location, preferred language), insurance type and member ID, urgency, the referring physician's name and NPI, and any notes. This information is Protected Health Information (PHI) under HIPAA and is transmitted only over encrypted channels through a vendor with whom we have a signed Business Associate Agreement (BAA).

Server logs

Our server records standard request metadata (timestamp, IP address hashed daily, opaque request ID, HTTP status). We do not log the body of any form submission. Logs are retained only for operational and security purposes.

Cookies and tracking

We do not use cookies on this website. We do not embed third-party analytics, advertising, or social-media tracking pixels. Cloudflare Turnstile, used to prevent bot submissions, runs without cookies in its invisible mode.

How we use your information

We use the information you submit only to:

  • Respond to your inquiry or schedule a callback.
  • Coordinate patient care and admissions when you make a referral.
  • Operate, secure, and improve this website.

How we share your information

We share your form submission only with the vendors required to deliver it to us, and only under contracts that bind them to confidentiality and (for protected health information) HIPAA. Our email delivery vendor transmits your submission to our staff. Our hosting provider routes the network traffic. We do not sell or rent your information to anyone, and we do not share it for advertising purposes.

How long we keep your information

Submissions are retained at our email vendor for approximately {retentionDays} days, and in our staff mailboxes for up to 90 days, after which they are deleted. Server logs follow standard operational retention. If you ask us to delete your information sooner, we will do so unless we are required by law or by your hospice care plan to retain it.

How we protect your information

All form submissions are transmitted over TLS 1.2 or higher. Our HIPAA-eligible email vendor encrypts submissions at rest. We restrict access to your information on a need-to-know basis. We do not store form submissions in any application database, and we never log the contents of a submission — only request metadata.

Your rights

California Consumer Privacy Act (CCPA / CPRA)

If you are a California resident, you have the right to know what personal information we have collected about you, to request that we delete it, to correct inaccurate information, and to be free from discrimination for exercising these rights. We do not sell or share your personal information for cross-context behavioral advertising, so there is no "Do Not Sell or Share My Personal Information" link on this site.

HIPAA

If you are a patient or the personal representative of a patient, you have additional rights under HIPAA — including the right to access, amend, and receive an accounting of disclosures of your protected health information. Those rights are described in our HIPAA Notice of Privacy Practices.

Children

This website is not directed to children under the age of 13. We do not knowingly collect personal information from children. If you believe a child has submitted personal information to us, please contact us and we will delete it.

Changes to this policy

We may update this Privacy Policy from time to time. The "Last reviewed" date at the top reflects the most recent revision. Material changes will be noted on this page.
