About this Notice
A HIPAA Notice of Privacy Practices is a document that all health care providers are required by federal law to make available to patients. It describes how the provider may use and disclose protected health information (PHI), explains the patient's rights under HIPAA, and tells the patient how to file a complaint if those rights are violated.
