Hospice is comfort-focused care for individuals with a life expectancy of six months or less. Rather than pursuing a cure, our team focuses on relieving pain, managing symptoms, and preserving dignity in the place you call home.

Every patient is supported by a team of specialists who coordinate care, anticipate needs, and walk with the family through every step.

  • Hospice Physician

    Oversees the plan of care and manages complex medical needs in partnership with your primary doctor.

  • Registered Nurse

    Leads symptom management, medication oversight, and family education at each visit.

  • Licensed Vocational Nurse

    Provides bedside nursing care, comfort measures, and continuity between RN visits.

  • Medical Social Worker

    Helps families navigate emotional, financial, and community-resource concerns.

  • Spiritual Care / Chaplain

    Offers non-denominational spiritual support honoring the patient's faith and tradition.

  • Hospice Aide

    Assists with bathing, grooming, and daily personal care with gentleness and respect.

  • Bereavement Coordinator

    Supports the family for up to thirteen months following the loss of a loved one.

  • Trained Volunteer

    Provides companionship, light errands, and respite presence so caregivers can step away.

My father passed away from advanced Parkinson's disease and was under hospice care with Eminent for quite some time. The nurses and staff at Eminent were some of the kindest and most compassionate people I have ever met. They were also incredibly efficient, professional, and excellent communicators (I could text them even at night or on weekends and get a response right away). They also provide seamless bilingual care — speaking Korean with my father while communicating clearly with me in English. They treated my father and our family with such warmth, dignity, and respect throughout the entire process. The care we received was so exceptional that after my father's passing, I felt compelled to leave a public review for this amazing hospice service. If you or a loved one need hospice care with bilingual support, I cannot recommend Eminent highly enough.
